As for the new enemies, they don't show up until the final part of the game, which is the literal Lost Chapter alluded to in the title. While all the other additions are seamlessly integrated, the final portion of the game is quite clearly an "extra" section, and contains an entire plot arc unto itself that's entirely new. Once you finish the final boss from the original game, a previously unseen hero makes contact with the guild, and before you know it your hero is off on a long journey by boat to the heretofore unexplored northern continent. Die-hards will note that this continent has always been on the in-game map. Now, you finally get to go there, which finally gives you somewhere to use that Sword of Aeons you had from the end of the game the first time through.
