If you have offended against any of the rules or "tenets" of the Dark Brotherhood, the next time you sleep, you will be confronted by Sithis. He is in a weakened form. You just have to kill him to get reinstated.

The second time you offend and get expelled from the Dark Brotherhood, Sithis will appear in a much stronger mode. Again, you have to take him on in battle and destroy him.

However, Sithis is indestructible. Should you offend a third time he will again come to you in your dreams. You have to kill him yet again in order just to survive. But this time there is no going back. You have been permanently expelled from the Dark Brotherhood, and there is nothing you can do about it.
