Suggestions:

Change:

Strength 18
Constitution 12

..alteration:

+1 Constitution (at level 4 for Toughness unless you go with the FB class as mentioned below.)
and Great Dexterity at level 21 (..again, without selecting the FB class.)

(..this delays Perfect Two Weapon Fighting by only one level, and none if you go the FB route.)

Veteran for you "history" feat.

Remove:

Monkey Grip
Improved Power Attack

..alterations:

Potential:
Exotic Weapon Proficiency
All the focuses and Specializations to either Bastard Sword or Katana.
(..2d6 vs. 1d10 isn't a huge difference. On the other hand -2 attack penalty isn't that different either.)

Toughness and Steadfast Determination at non-fighter feats as early as possible.

Two Weapon Defense & Improved Two Weapon Defense (..put these late in the build, but before level 21.. a +2 AC doesn't make as big a difference as killing off opponents more quickly.)

Blindfight

Better yet:

Consider getting your Toughness for "free" from the class Frenzied Berserker AND Supreme Cleave (i.e. 2 levels only of FB). (..you also wouldn't need to spend an additional point in Constitution at level 4). This class can be inserted as early as levels 7 and 8 to select Steadfast Determination at level 9.

Also:

Always consider the potential of the Ranger class instead of Fighter and strained Dexterity points.

After looking through *when* feats are selected.. Katana or Bastard sword doesn't seem that different (.. -2).

Beyond the problems with Constitution and saves generally, skills are pretty non-existent in this build, and that does factor-in with a melee fighter - particularly with spotting hidden or invisible opponents. (..if you can't target them, you can't hit them.)

Finally:

Your epic feats are also off (unless they are wrong on the "builder" site).. you don't have that many. You don't end-up with a 31 in Strength. The best that can be achieved is 28 with Perfect Two-Weapon Fighting, Epic Focus, Epic Weapon Specialization, and Epic Prowess. (..no Armor Skin.)

thanks Scottg, i've read lots of your posts, you change everything i've planned :D

Actually i don't want to rely on "Critical Hits" and "Items". I think, a good character should hit hard and fast with simple weapons. If i found a magically better items, or if i use buffs, it will be just "an" increase.

Here is my new char (i will call her Drisst) :D

Yuan-Ti Pureblood
Chaotic Good
Veteran for "history" feat ;)

Ranger (21), Fighter (4), Frenzied Berserker (5)

Str 17 (24)
Dex 18 (+4 dex. bonus is enough for good armors)
Con 14
Int 12
Wis 8 (if i increase this with items can i use spells?)
Cha 11 (odd numbers are good for +1, +3, +5 cloaks)

Hit Points: 358
Armor: 19 (i can't find another way to increase AC, any advice?)
Spell Resistance: 41 (this is awesome)

Main Hand: +36 / +31 / +26 / +21 / +16 / +11
Off Hand: +36 / +31 / +26 / +21 / +16 / +11

Fortitude:24
Reflex:19
Will:12

Skills: (i don't know if i need other ones, i can decrease intimidate and increase a new one)
Appraise:16 (17)
Bluff:16 (16)
Diplomacy:16 (15)
Intimidate:24 (24)
Lore:33 (34)
Tumble:10 (14)
Hide:15 (19)
Move Silently:15 (19)
Use Magic Device:16 (16)
Spot:15 (16)
Listen:15 (16)

lvl 1 ranger (1), fav.en:undead, luck of heroes (+1 to sav.thr. & AC)
lvl 2 fighter (1), exotic weapon prof. (katana, i love those)
lvl 3 ranger (2), 2weap.fight.comb.style, able learner (earliest possible for skills)
lvl 4 fighter (2), weapon focus (katana)
lvl 5 ranger (3), (ranger: toughness)
lvl 6 fighter (3), steadfast determination (thanks for this :) )
lvl 7 ranger (4), companion: panther (i will cal her guven :P )
lvl 8 fighter (4), weapon specialization (katana, now it has +2 damage)
lvl 9 ranger (5), fav.en:humans, dodge (extra AC doesn't hurts)
lvl10 ranger (6), (ranger: impr.2Weap.Fighting)
lvl11 ranger (7), (ranger: woodland stride, speed is good)
lvl12 ranger (8), 2weap.defense (+1 AC)
lvl13 ranger (9), (ranger: evasion)
lvl14 ranger (10), fav.en: outsiders
lvl15 ranger (11), improved 2weap.defense (+2 AC) (ranger: Gr.2Weap.Fighting)
lvl16 ranger (12),
lvl17 ranger (13), (ranger: camouflage)
lvl18 ranger (14), improved crit:katana
lvl19 ranger (15), fav.en:beasts
lvl20 ranger (16),
lvl21 ranger (17), power attack (ranger: Hide In Plain Sight (outdoors))
lvl22 ranger (18),
lvl23 ranger (19), cleave
lvl24 ranger (20), fav.en:monstrous humanoids
lvl25 ranger (21), great cleave (ranger: Perfect Two-Weapon Fighting)
lvl26 fr.berz (1), (frenzy:+6 STR & extra attack for 5 rounds, massive power attacks and supreme cleave)
lvl27 fr.berz (2), improved power attack (+Supreme Cleave)
lvl28 fr.berz (3), (+frenzy)
lvl29 fr.berz (4), extend rage (now it lasts 5+5=10 rounds) (+Deathless Frenzy)
lvl30 fr.berz (5), (+Frenzy, +Enhanced Power Attack)

well, i think this one is better ;) thanks Scottg

edit: maybe, i will take fighter levels later;
1- for removing xp penalty
2- for gaining extra off-hands attacks 4 lvls earlier (with perfect 2weap.fight.)

You don't need more than 17 in Dexterity (for Improved Two-Weapon Defense).

Reasoning:

You can only wear light armor.

Mithral armor is the best:

Mithral - NWN2Wiki, the Neverwinter Nights 2 wiki - Races, classes, skills, and more

When you compare the AC value and the amount of Dexterity bonuses the maximum is a net value of +10:

either Chain Shirt or Breastplate - they both equal a max of +10.

You can augment your Dexterity from any number of sources, the easiest being a Cat's Grace potion. This adds +4 to Dexterity.

+4 and a 16 in Dexterity gets you all you'll need. (i.e. net 20 or +5 Dexterity bonus for Mithral Breastplate.)

You would of course add-in +1 to Dexterity to get it to 17 for Improved Two-Weapon defense.

Anything more than that is largely a waste, and is better spent on Strength.

There are other ways to improve AC. For your build it's pretty much limited to Barkskin (2nd level Ranger spell) or items with AC enhancement, or the inclusion and use of Combat Insight and Improved Combat Insight (..though I don't remember if this works with Two-Weapon Defense or not).

Other methods include:
Other Spells..
Polymorphing.
Cleric/Blackguard/Paladin: Divine Shield.
Monk Wisdom bonus.
Sacred Fist AC bonus.
Red Dragon Disciple AC bonus.
Pale Master AC bonus.
Invisible Blade Intelligence bonus.
Dualist Intelligence bonus.
Bard: Inspire Defense.
Shadow Dancer: Shadow Evade.

All of these present problems for your build, and most are completely out of the question. :o

I went through several different builds, trying for decent saves, good AC, good damage and attacks, and of course two-weapon Katana fighting.

(..exceptions were: late selection of Perfect Two Weapon fighting, no "Use Magic Device", mediocre criticals, and no dialog skills or healing.)

It is not a "better" build, (and in some respects it isn't as good), but it is a bit different (..with a higher AC).

Two-Weapon Katana Fighter | NWN2 Character Builder

A few things to note:

1. It does have Supreme Cleave - and at a decent point in the build.
2. Steadfast Determination is also selected at a decent point in the build (..neither "good" nor "bad", and reasonably well balanced when compared to other fighting skills.)
3. It's a Human, so no Spell Resistance.. BUT no 2-level penalty either.
4. Saves are enhanced +1 against Spells via every 5 points in Spellcraft. (..this effectively increases the build's saves - throughout the life of the build.)
5. It's a Bard, so don't forget to use Inspire Courage for the +1 attack and +1 damage.
6. You can use various types of armor (without restriction) at any point in the build to get the best AC possible.
7. Hit-points are a bit higher with this build.. so it can take a bit more of a beating and still survive.
