I can just keep on playing NWN2 with my old characters, but my new characters won't come very far in the game 'cause when I pick up the first silver shard (you know, with Bevil in the lizardfolk caves) a conversation should trigger where he offers to bring me back to West Harbor, but it doesn't. When I travel back there myself (with the shard of course) Duncan asks why I have come back empty-handed!
About half a year ago I read a thread of a guy/girl who had the same problem, but I can't find it anymore. It suggested reloading a previous save, I remember, but this doesn't work. Starting with another new character doesn't work either, ALL my new characters have the same problem.
If anyone here would know how to solve this I would be VERY grateful, 'cause all the bugs in NWN2 are starting to annoy me.
(excuse me for my perhaps crappy english, it is not my native language)
I got it patched till version 1.10 or something, but do you need Storm of Zehir to have it FULLY patched? 'Cause I don't have SoZ... And the reinstalling is actually the problem, 'cause I reinstalled it before to (succesfully) solve another bug and then this problem showed up.

No, you don't need any of the additional modules as far as I know (..neither SOZ nor MOTB).
Basically you keep "updating" until you can't update anymore. IF you've done that then you are patched as far as you can go.
..and sadly yes, some installs are the problem - particularly with conflicts from some basic Windows services and other programs that are running in the background. On the other hand most games today seem to have this problem.
