I am supposed to light this beacon. I get through the first floor but the 2nd floor is reached by a ladder. How do I climb it?

If you keep working you way through the level, you should find a staircase. If you've mapped out the entire level, you should be able to see on the map screen where the exit is.
Crush enemies, see them driven before you, and hear the lamentations of the women.

The ladders aren't the transition to the upper levels. You need to go through big doors to move up, so just move onwards in the area.

If you check your map you should see a white circle on a doorway. It will tell you it's the "stairs to 2nd floor/3rd floor/etc".
