Not sure about the patches. I haven't tried any of them, though I am tempted to check them out just out of curiosity.
Anyways, I know that this post is coming extremely late (7 years late, but I just picked this game up again). If you aren't running any of the unofficial patches, this is how to find the blade.
When you enter the hollowbrook hotel, you go down the stairs and turn to your left around the corner. In the first room, there are a sabbat gangrel and two thugs. Passed those targets you enter a hallway with an intersecting hallway. Move down the hallway passed the intersection. There is a room on your left hand side, directly across from a room with a burning trash can. The Tal'mahe'ra Blade is leaning against the wall in that room. Not sure what level of inspection you need to spot it, but I know I didn't see it glowing on any of my play throughs, one had an inspect at 6.
