When I cast Ghostly Visage and Ethereal Visage
or Stone skin + Greater Stone skin
or Endure Elements + Protection from Elements

Do the effects stack up?

nope! they dont...
I once had a little teaparty, this afternoon at three, twas was very small, three guests in all; I, myself, and me. myself ate up the sandwhiches, while i drank up the tea. twas also i that ate the pie,and passed the cake to me :D

But then, when I use improved invisibility+Ethereal Visage, do I get 75% concealment bonus?

Nope, if they would a monk with empty body and improved invisibility would get 100% consealment (and with the self-conseal feat get 150%)
