I think it was just a patching issue, i updated to the next patch and swapped her in and out of my party and as far as i can see it's stopped happening. Thanks for the help!

Hi, I'm having a bit of trouble as ever since Qara has got prismatic spray she keeps casting it and killing all the other characters including my own in the party.
