There are two ways to break through the floor planels. The first is to use the use "e" key. The second is to use a firearm and shoot the panels...takes about 3-5 shots depending on the gun.

I've had a similar situation in that I'm never able to find Tin Can Bill. The walkthrough says I should find a bag lady by one of the fires. The only NPC I find that is a bum and talks on the street is a male and he doesn't appear to have a name. He does ask for the $20 bucks, which I then find out ...

Actually, there's at least one in each of the four areas and in the Aslyum there are two (one of them is upstairs).

First verify that you have ammo.

Second make sure that the weapon is in use...i.e. press 2 and make sure that weapon is in your hand.

Last...press the letter "R".
