Sneak attacks... when do they kick in?

I'm wondering when sneak attacks kick in. I know if you are in stealth mode and attack somebody from behind you get the bonus. I also know that a stunned enemy gets the damage.

I was told that sneak attacks apply with a Guardian's Force Jump. Is this true? (cuz it would be awesome)

What about with ranged weapons?

What about stealthed attacks from the front?

Okay, here is where sneak attack applies:

1)First attack when stealthed
2)When you are either behind or flanking an emeny
3)Guardian's force jump (yes it is awesome)
4)Usually the first round of combat if you attack first
