Anyone figure how tool set mods might integrate with mulitplayer? Is this a MMORPG like WoW, or is it a small scale multiplayer like Guild Wars?

Tool set mods for NWNII will integrate with multiplayer in the same way as in the original NWN. I think you are referring to haks and and not user created modules. Haks need to be downloaded by each player in a multiplayer game so that everyone has the same hak files. Many custom modules require haks as well.
NWN is not at all like WOW, and really not like Guild Wars either - as multiplayer games in NWN are hosted by individuals and not the game company. NWN multiplayer has three basic incarnations:
1. Multiplayer sessions hosted by players playing either the main storyline or a custom module
2. Multiplayer sessions hosted by a DM where the DM interacts with characters like in traditional PnP. (there are rumors that the initial release of NWN2 will not include a "DM Client" but it would most likely be included in a update patch soon after release)
3. Persistent Worlds hosted by the PW group where players log in and create and roleplay characters in a custom built environment.
