Since gamebanshee.com has not much info about how to get a specific class I suggest we post it here. I know there are some people here who got mixed classes I didn't even encounter in the game (like monk) but I know they exist 
Just say class name, where to join it and next tiers.
Like this:
Fighters guid
fighter->knight->lord
Temple of order
adept->paladin->crusader
