What part of NwN is it? The orginal game or one of the expansions? Also give us some more info before we can answer your question

Are you in Wanev's summoning chambers?
If so, I think the heart has to be taken to a brazier near the entrance to the the chambers and used. If this is succesful, the portal should collapse in an explosion (like from a fireball spell).
You could also check with Wanev to see if he is happy that the quest is finished.
