Where does this dust come from? Why is it radioactive?

It comes from the explosion and it is radioactive due to it beeing an explosion of nuclear material.

If you have ever seen a picture of TV-shot of a nuclear bomb, then you will have seen the "mushroom cloud" that appears at explosion. That cloud consist of dust from the effects of the explosion, and because the explosion is nuclear it causes radiation which thus will cause the dust to be radioactive.

In the Fallout Universe - many such explosions have taken place (nuclear war/apocalyptic war) and such dust have been spread around in the atmosphere.
