How did you manage that!
How did you manage to get into the Vale of Ruin? Did you jump over the hills! Never a good move as it can leave you stuck in places. *Just saw the climing post, it seems you can go for a jolly in the hills quite easily, not sure how high you acrobatics needs to be for this.*
the only way I've gotten in and out of the Vale of Ruin is via the portals which require a moonstone, which I never picked one up until halfway through the game.

Find the moonbridge and go south from there; there is a good climbing spot in that area. Once you are at the top, it's east to the forbidden lands.

I've had moonstones disappear before, too. Just always make sure you only have one, and you should be ok.

Directly north of the Draedoth temple, there's a way up into the mountains and out of the Vale of Ruins that doesn't require backflips. (See the map I posted in the climbing thread.) Also, make sure your mouse works properly.

I picked up 4 moonstones but by the end I only had one left, I never even noticed them disappearing.
Wulf's climbing threads are pretty detailed so hopefully that will get you out of the Vale of Ruin.
I recently picked up the game and it came with no map, so after about getting halfway through the game finding the map on this site was very helpful.

Usually, you drop the moonstone every time you pick up a new one. The game understands that you only need one moonstone. I think the problem occurs when the game doesn't automatically drop the moonstone for you. When your inventory fills up, the game begins to automatically drop "junk" items. Sometimes, though, it messes up, and drops other items. I lost a Kama of the Moon that way once, too.

Yes, only one moonstone will show up under the 'keys' tab, the others will be in 'junk'. Since the 'junk' is set up to keep the eighteen most valuable items, the surplus moonstones will get pushed out first as the 'junk' fills up, since their value is 0. The same happens to surplus dragon eggs.
