Star Gentle Uterus is an energy blast that Star Maker grabs from his uterus (literally) and fires off. It is quite gross

There's two explanations here. One is very disturbing.

In the Sailor Stars anime, the Starlights are male, but turn into females when they go from 'civilian' form to 'sailor senshi' form. It is in the Sailor Senshi form that Maker uses her Star Gentle Uterus attack.

In the Sailor Stars manga, the Starlights are girls pretending to be guys, while being 'civilians.' Whether civ or senshi, though, in the manga they're always girls. Naoko-san doesn't like what the anime makers did to her Star Senshi, but she doesn't have much control over it.

Technically, it'd be her Star Gentle Uterus, since she's female when using the attack.
