This page might be of help. You can only brew potions when you meditate.

were do i get the stuff from and i need a vial can i get it from a herbalist

Potion Bases can be bought at Herbalists, and also at inns.

You can purchase the other ingredients (other than the specials, such as the Frightener's Claw) from various sources, and also simply pick them from plants/monsters (Plants need the Herbalism skill).
