Are you using your silver sword? Did you try using the Aard sign?

Try fast combat style. If enemies are dodging a lot it is because you are using strong style on a fast enemy. If you are hitting and doing minimal damage then it is because they are a slower more armoured enemy. Use strong style on them.
This game requires you to think things through a bit, if one combat style is not working try another. Remember also your silver sword is useless against humans elves and dwarfs.
