| It is your base level in a skill that determines your level of mastery in it. That is, it is the value printed in blue, not green or red. Accordingly, if you use enchantments to up your level from, say, 73 to 78 (i.e. from journeyman to expert level) you will not count as being an expert of that skill, nor will you get the skill perks (power attacks, etc.) associated with the expert level.
Each trainer has a limit to the level to which he can train you. There are only a few master trainers scattered around Cyrodiil. Finding one is considered a quest in itself. Ask around about rumours and you have a fair chance of getting tipped in the right direction.
And no, you don't really need a master trainer. You can always advance the natural way. However, if we're talking minor skills, though theoretically possible, in practice it is very hard to get to expert, let alone master. But it also depends on what specialization you chose (compat, magic, stealth).
