My companion Khelgar seems to be Sands familier how do I fix this?

Latest patch should fix all party and summon bugs.
Did you run the patcher?

Most odd. Did you try returning to your party hub after patching?
It might not be properly reset until you do.

If this doesn't work you're probably best of loading an earlier save before this bug occurred. The patch should at least prevent it from happening again/
