I am not that familiar with the D&D3 rules, so wait for fable or someone else to stop by and answer your first two questions, but I
can answer your last one.
The minimum number of skill points you can get per level is 1, regardless of a character's INT. I've drained out the INT of two of my characters (one's a fighter and one's a barbarian; neither needs a high INT) and they each get 1 skill point when they level up.
