How do you read holo-discs?

I feel kind of dumb for asking this, but I couldn't figure it out... If I try to equip it and then use it I get the little X where I get to choose what to use it on... When I click myself I get the message "nothing happens". I know I'm supposed to use my pip-boy but I've no idea how... help?

How did you do it then?

Right click to bring the spearhead up. Then left click and hold on the holodisc. Then highlight the icon of the open hand and release.
