Start a new game, choose a different class, and use different NPC's. Bump up the difficulty as well. The game will basically be the same but you will definitely come across some new things.
ToB was okay. I'm playing through TDD currently and haven't noticed too many changes. I've only found one new area and have yet to explore it. There are new NPC's but I am only using one of them.

| | | The Throne of Baal add-on sequel is pretty linear, but it does add some nice story background, higher level spells and abilities, a nice class, and a new party NPC. When that ends, you literally get a completion to the tale, complete with summary of the lives of your comrades and yourself. It can vary, according to your actions.

Though it would be cool if you start playing as an other child of bhaal, then in the end you are confonted with your old self... Nah, that will never work.
