there is no physical door, you talk to saemon, agree to what he proposes, and he teleports you outside, then walk to the edge of the map, and your back in brynnlaw...
The door out of Spellhold is the same one you came in, though IIRC, you'll need Saemon's help to get out. Once you're out, there's nothing stopping you from re-entering.
