Helm of Balduran

I read somewhere in one of the forum posts that by the end of Irenicus' Dungeon that you could have picked up 2 Helms of Balduran. Was that a misprint? Or was it using some script tweaking? Or am I, as my title suggests, a dumbass who isn't looking aroung hard enough...

Normally there is just one helm of Balduran.
As for the second, either somebody has used somekind of bug, script or cheat to get it.
