It doesn't make much of a difference. Although the manual suggests you only get one attack with your off hand weapon, in fact you will get 1 extra attack and your total number of attacks will then be split evenly with any extra going to your primary hand. With a high level fighter using the boots of speed, this means you will get 2 with each unless hasted.
So, the only difference is the -2 to Thac0 with the off hand (with 3+ in dual wield). Much of the time this won't matter since you'll only miss on crit hits anyway. However against an opponent with very good AC, it can make a slight difference. Since FoA actually does more damage (since both weapons get the Str bonus) and also gives a chance to slow you want that as your primary.
Heh...just re-read the example. In this case you definitely want the Flain in the right hand, like Xandax said.
