You need to go and meet 2 guys (one is called Marek IIRC) that will come up to you and talk to you.
I can't remember if they come up to you on any map - or there is a specific map, I remember I met them if you enter the North-East map from the south.
Later when you enter one of the central maps (looks like somekind of town fair) one of the men will approch you again and talk to you wich triggers the quest.
This quest will eventually lead to the Tome of Wisdom wich you then can aquire if you play your cards right.

Dunno how "doomed" you'd be... I did the quest like I was supposed to and then waited with drinking the antidote to see what happened. Finished the game without ever touching it.

No, the loss of the Tome doens't mean you're doomed. The High Priestess of Umberlee has the scroll of geas you need for the quest. If she doesn't give the scroll voluntary you could just kill her. No reputation penalties.
I think you should kill her anyway: she shouldn't be rude to the magnificent warrior that you are.

Completing the quest removes the poison - you don't have to physically drink it. Dumb eh?

You can pickpocket the tome *after* you give it to the Priestess - making for 3 wis tomes in BG & ToSC

I useally always kill her anyway after giving her the Tome, so I can get it back.

I'm just starting Chapter 7. Out of curiousity, are there any other tomes in the expansion?

Durlagh's Tower is in the expansion (TOSC). To my knowledge three tomes of wisdom in total.
