Items Glitch

Is anyone else having problems with picking up items sometimes? I have plenty of pack space (not even close to filling it), and I've come across a glitch in the system where I am randomly unable to pick up an item.

It usually seems to happen with health or mana bottles, but unfortunately it is making my character unable to pick up the Shard of Torment at the moment. I obviously can't continue without picking it up and giving it to Gol Nazyn...

Any thoughts?

Try dropping some potions or items, picking up the shard, giving it to Gol Nazyn then returning to retrieve the potions (or items) you drop.
