In ShadowKeeper, select your character, go to the Innate tab, click "Add Spell"; Slayer Change is listed under Innate, Level 1. Add it to your character (button at the bottom of that box) and you should be able to slayer-change again.
On the ability disappearing: Maybe you used Cloak of the Wolf of that cloak that turns you into a Troll/Puddle/etc
while in Slayer form?
