I should tell you that this question has been examined exhaustively, here--so we always recommend to everybody, check the database using our search engine, first. Try mindflayer, illithid, vampire, and knock yourself out with over 50 suggestions on how to deal with 'em.
In areas where the database doesn't help, see if the main site has a strategy guide for the game, and if it does, whether that guide can help. Then, if you can't find what you want, post to your heart's content. The site owner doesn't want duplication threads, however, because of the way it's slowed down other boards. So I'm afraid I'm going to have to close this one, too--but let us know if you have further questions.
Oh--as regards the Slayer, that's covered elsewhere, too. But basically: you have a limited amount of time to remain in that form, before you start to die. You have to turn back, which you can do, manually.
