Given the choice you have as Revan in KotOR I, Zaalbar isn't likely to be in a third game. They might do something like they did with Carth in II, where if you chose Revan as a dark sider, Zaalbar will not be in III, but if Revan was light, then Zaalbar can be.
Also, since at the end of II you either choose to be light or dark, that determines whether Malachor V stays intact or not. Since that's really a rather monumental decision, I think it's highly unlikely Malachor V will be in III, although it may be mentioned.

George Lucas, in an interview in Starlog, specifically said he didn't give a rat's bum about the novels (I'm paraphrasing), he didn't read them, he doesn't base anything of the movies off them, and he expects the authors to write their books based on his vision alone. As such, I figured he (Lucas) would have his own reason for why the dark side cave on Dagobah existed.
