Like all tough battles there is a trick. For the twisted rune, which I completed in Chapter 2 (character level 12-13), it was kind of neat. When I got there, I forgot that I had already used all of Yoshimo's traps. Crap! I almost gave up when I decided oh, what the hell! So here are some special things I used ...
Armor: Shield of Balduran
Spells: Death Fog, Emotion:Hopelessness
So what I did was activate the lich and then quickly walk away before he begins his evil rant. Then my mage cast Death Fog making sure the lich was in the cloud. This works despite the lich's natural resistances (you can also use Inciniary Cloud, and I'm sure some others, but Cloud Kill WILL NOT work)***. So I waited until the spell ran out (he was down to 'Badly Injured' by the way) then I gathered my melee folks around where he will teleport to (a slight cheese tactic, I know) making sure Jaheira (who had the shield of Balduran) was closest to the doorway.
Next, I sent someone out to talk to the lich after which he teleports in and I start pummeling him with the melee fighters and backstabbing with Yoshimo. Of course he begins to trigger all his protections so my I have Aerie cast breach, and Keldorn use his SUPERB Dispel Magic ... etc, etc. until he is slain.
Meanwhile, I am making sure not to move out too far into the main chamber so I stay out of reach of the Vampire and the Mage. Unfotunately, you cannot keep away from the Beholder and the Fighter (that's why I had Jaheira with the Shield of Balduran closest to the doorway). A few reflected rays and some extra 'help' from my mages and the beholder was toast. I don't even remember how I killed the Fighter but compared to the others he was trivial.
After that I took my time with the Vampire and the Mage. I happened to get lucky and I used Emotion:Hopelessess on the mage before I even saw her, so I just walzed up and beat the crap out of her and took the staff ...
***Note, make sure that whatever you do YOU DO NOT KILL the lich before he does his evil rant and teleports into the antechamber or the other bad guys will not spawn and you'll be trapped in that damn place forever!!!
