After you try to run the exe, check task manager if there is process called "fallout2.exe" (or similiar). If there is, try to end it and run the game again. Of course, before rerunning the exe be sure there is no processes with that name running, as I have seen couple of times that for some reason I had two processes with that name running, and the game didn't start anymore.

After that, you could try what you said you haven't, it just might work.

Have you ran your virus scan lately? That problem could be caused my virus. Also, you should check that the game hasn't been blocked by your antivirus and/or firewall software. Happened to me couple of times with Fallout 1, but this depends on fully your antivirus/firewall software.

Lastly, did you try this method? There could be something running which blocks Fallout 2 entirely.
